- INTRODUCTION AND PURPOSE
This Consumer Health Data Privacy Notice (this “Notice”) explains how Advanced Sonography Screening LLC, a Pennsylvania limited liability company (“Advanced Sonography Screening,” “we,” “us,” or “our”), collects, uses, stores, and discloses certain personal information that identifies, relates to, describes, or is reasonably capable of being associated with a consumer’s past, present, or future physical or mental health status (“Consumer Health Data”).
This Notice supplements our Website Privacy Notice and applies when you:
- Schedule or receive cardiovascular screening services;
- Complete intake, consent, or pre-screening forms;
- Use our scheduling tools, customer portal, or communications channels;
- Communicate with us regarding screening services; or
- Otherwise interact with us in connection with health-related services (collectively, the “Services”).
This Notice is intended to address obligations under state consumer health data privacy laws. It does not replace medical confidentiality or professional obligations that may apply in other contexts.
Where required by law, we obtain your affirmative consent before collecting or processing Consumer Health Data.
- CONSUMER HEALTH DATA WE COLLECT
Depending on how you interact with us, we may collect the following categories of Consumer Health Data:
- Screening and Results Data
Information relating to cardiovascular screening services you select or receive, including screening type(s), technical findings, measurements, and screening results generated through the service.
- Intake and Pre-Screening Questionnaire Data
Information you provide through intake forms or questionnaires, including medical history, risk factors, symptoms, or other health-related information relevant to the screening services requested.
- Appointment and Service-Related Information
Information related to scheduling, attendance, service location, service preferences, and other service-administration details that may be associated with your screening activity.
- Transaction and Payment Information
Information related to the purchase of services, such as products or services selected, transaction date and amount, and payment confirmation details. Payment card information is generally processed by third-party payment processors.
- Communications Containing Health-Related Information
Health-related information you choose to share when communicating with us by email, phone, text message, webform, or other channels in connection with Services.
- HOW WE USE CONSUMER HEALTH DATA
We use Consumer Health Data only as permitted by law and for the following purposes:
- Providing and Administering Services
To provide, deliver, and administer screening services you request, including appointment scheduling and confirmation, preparation instructions, delivery of results, and customer support.
- Operations and Business Administration
To operate our business and internal systems, including billing, accounting, recordkeeping, internal reporting, quality assurance, troubleshooting, system maintenance, and service improvement.
- Security, Fraud Prevention, and System Integrity
To protect the security of our systems and data, detect and prevent fraud or unauthorized activity, and enforce internal policies and procedures.
- Legal and Regulatory Compliance
To comply with applicable laws, regulations, lawful requests, subpoenas, and legal processes, and to protect our rights, safety, and property and those of others.
- Analytics and Service Improvement
To analyze and improve our services, operations, and user experience. Where required, we use de-identified or aggregated data.
- Communications (Where Permitted)
Where permitted by law and consistent with your choices, to communicate with you regarding services, availability, and related information. Marketing consent is not required to receive Services.
- Information Related to Clinical Trial Opportunities (With Consent)
If you provide your affirmative consent (at a screening event, through our Website, intake forms, authorization forms, or otherwise), we may review certain Consumer Health Data derived from your Screening and Results Data to determine whether you may be eligible for participation in clinical research studies or clinical trials offered by trusted third-party research organizations.
Participation in clinical trials is entirely voluntary, and your decision to participate or decline does not affect your ability to receive screening services from Advanced Sonography Screening.
- HOW WE DISCLOSE CONSUMER HEALTH DATA
We do not disclose Consumer Health Data except as described below, as authorized by you, or as permitted or required by law.
- Service Providers
We may share Consumer Health Data with vendors and service providers that perform services on our behalf, such as IT support, secure data storage, scheduling platforms, payment processing, billing support, analytics, communications services, accounting, auditing, and tax services. Service providers are contractually required to protect the information and use it only for authorized purposes.
- Quality Review and Clinical Oversight (Where Applicable)
We may disclose screening-related information to qualified personnel involved in quality assurance, technical review, or clinical oversight activities consistent with the nature of the Services and applicable law.
- Your Authorized Healthcare Providers
If you request that we share screening results or related information with your physician or other healthcare provider, and you provide the necessary contact information and authorization, we may disclose such information for your benefit.
- Clinical Trial Researchers
If you provide consent and are determined to be eligible for a particular clinical trial, we may disclose limited and relevant Consumer Health Data on your behalf to trusted third-party research organizations or their authorized representatives for purposes related to clinical trial screening, enrollment, or participation.
If you enroll in a clinical trial, you will receive separate privacy notices and informed consent documents directly from the applicable research organization(s), which will govern their independent use of your information. Advanced Sonography Screening does not control and is not responsible for the privacy practices of independent research organizations.
- Corporate Transactions
In connection with a merger, acquisition, financing, reorganization, bankruptcy, dissolution, or sale of assets, Consumer Health Data may be disclosed or transferred subject to applicable legal requirements and appropriate safeguards.
- Legal Requirements and Protection of Rights
We may disclose Consumer Health Data if required by law, court order, subpoena, or other legal process, or if we reasonably believe disclosure is necessary to protect the rights, safety, or security of our business, customers, personnel, or others.
- Sponsored or Organizational Arrangements (If Applicable)
If services are arranged through an employer, organization, or sponsor, we may share limited administrative information only (such as confirmation of attendance or payment details) and not screening results, unless authorized by you or required by law.
No Sale of Consumer Health Data: We do not sell Consumer Health Data.
Targeted Advertising: We do not use or disclose Consumer Health Data for targeted advertising where prohibited or without required consent.
- YOUR CHOICES AND WITHDRAWING CONSENT
Where we rely on your consent to collect, use, or disclose Consumer Health Data, you may withdraw that consent at any time by contacting us using the information below.
Withdrawal of consent does not affect processing that occurred before withdrawal and does not require deletion of information we are legally required or permitted to retain (e.g., billing, compliance, or dispute resolution).
- YOUR RIGHTS UNDER CERTAIN STATE PRIVACY LAWS
Depending on your state of residence, you may have the right to:
- Request access to Consumer Health Data we maintain about you;
- Request correction of inaccurate information;
- Request deletion of certain Consumer Health Data (subject to exceptions);
- Request a list of certain disclosures;
- Revoke consent where consent is the basis for processing; and/or
- Request limitations on certain processing.
Verification
We may require reasonable identity verification before responding to a request. Verification may include confirming contact details. For correction requests, supporting documentation may be requested.

If a request is denied and applicable law provides an appeal right, you may request an appeal using the contact information below.
- DATA SECURITY AND RETENTION
We maintain administrative, technical, and physical safeguards designed to protect Consumer Health Data against unauthorized access, use, or disclosure. However, no system can be guaranteed to be completely secure.
We retain Consumer Health Data only as long as reasonably necessary for the purposes described in this Notice or as required or permitted by law. Information may remain in backups for a reasonable period.
- CHILDREN’S INFORMATION
Our Services are not intended for individuals under 18 years of age. We do not knowingly collect Consumer Health Data from minors. If such information is identified, we will take reasonable steps to delete it.
- CHANGES TO THIS NOTICE
We may update this Notice from time to time. When changes are made, we will post the updated Notice on our website and revise the Effective Date. Continued use of our Services after changes become effective constitutes acknowledgment of the updated Notice, to the extent permitted by law.
