This Notice describes how medical information about you may be used and disclosed and how you can access this information. Please review it carefully.
Advanced Sonography Screening LLC (“Company,” “we,” “us,” or “our”) is required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to maintain the privacy and security of your Protected Health Information (“PHI”), provide you with this Notice, follow its terms, and notify you in the event of a breach of unsecured PHI. PHI includes information that identifies you and relates to your health status, screening services provided, or payment for those services.
Advanced Sonography Screening LLC provides preventive cardiovascular screening services only. Our services are non-diagnostic and are not medical treatment. They do not establish a physician-patient treatment relationship with the Company. Screening data is collected and transmitted to an independent licensed physician for review and written interpretation. You are responsible for consulting your personal healthcare provider regarding any findings or follow-up care.
We may use and disclose your PHI without your written authorization to perform screening services, collect cardiovascular risk assessment data, transmit screening information to an independent reviewing physician, receive and document the physician’s interpretation, provide your screening report to you, and send results to a healthcare provider you designate with proper authorization. We may also use PHI for payment purposes related to self-pay transactions, receipts, and secure processing through third-party payment vendors. We do not routinely bill insurance for screening services.
We may use PHI for screening operations, including quality assurance, staff training, compliance reviews, credentialing independent physicians, risk management, fraud prevention, and business management activities. We may share PHI with trusted third-party service providers such as scheduling platforms, secure imaging and reporting systems, cloud hosting providers, IT vendors, and cybersecurity providers. These entities are required to protect your PHI through written Business Associate Agreements.
We may disclose PHI when required by federal or state law, in response to court orders or subpoenas, for public health reporting, for health oversight activities, or to prevent a serious threat to health or safety.
We will obtain your written authorization before using or disclosing PHI for marketing involving financial remuneration, selling PHI, or using PHI for research that is not de-identified or otherwise permitted by law. You may revoke authorization in writing at any time, except where we have already relied on it.
Screening results may be delivered through a secure patient portal, encrypted electronic communication upon request, or to your designated healthcare provider with written authorization. We will not transmit unsecured PHI electronically unless you acknowledge and accept the associated risks.
You have the right to request access to your PHI, request amendments to inaccurate information, request restrictions on certain uses or disclosures, request confidential communications, receive an accounting of certain disclosures, and obtain a copy of this Notice at any time.
We may use or disclose de-identified health information, as defined under 45 CFR §164.514, for quality improvement, research, and statistical purposes. De-identified data does not contain information that can reasonably identify you.
We implement reasonable administrative, technical, and physical safeguards to protect PHI, including encryption, secure cloud storage, access controls, staff training, and cybersecurity protocols. However, no system can guarantee absolute security.
